Over a Million Healthcare Devices Hit by Cyberattack

 


Despite the swell of cyberattacks changing the global threat landscape, Indian healthcare has become one of the most vulnerable targets as a result of these cyberattacks. There are currently 8,614 cyberattacks per week on healthcare institutions in the country, a figure that is more than four times the global average and nearly twice that of any other industry in the country. 

In addition to the immense value that patient data possesses and the difficulties in safeguarding sprawling healthcare networks, the relentless targeting of patients reflects the challenges that healthcare providers continue to face healthcare providers. With the emergence of sophisticated hacktivist operations, ransomware, hacking attacks, and large-scale data theft, these breaches are becoming more sophisticated and are not simply disruptions. 

The cybercriminal business is rapidly moving from traditional encryption-based extortion to aggressive methods of "double extortion" that involve stealing and then encrypting data, or in some cases abandoning encryption altogether in order to concentrate exclusively on exfiltrating data. This evolution can be seen in groups like Hunters International, recently rebranded as World Leaks, that are exploiting a declining ransom payment system and thriving underground market for stolen data to exploit its gains. 

A breach in the Healthcare Delivery Organisations' system risks exposing vast amounts of personal and medical information, which underscores why the sector remains a target for hackers today, as it is one of the most attractive sectors for attackers, and is also continually targeted by them. Modat, a cybersecurity firm that uncovered 1.2 million internet-connected medical systems that are misconfigured and exposed online in August 2025, is a separate revelation that emphasises the sector's vulnerabilities. 

Several critical devices in the system were available, including imaging scanners, X-ray machines, DICOM viewers, laboratory testing platforms, and hospital management systems, all of which could be accessed by an attacker. Experts warned that the exposure posed a direct threat to patient safety, in addition to posing a direct threat to privacy. 

In Modat's investigation, sensitive data categories, including highly detailed medical imaging, such as brain scans, lung MRIs, and dental X-rays, were uncovered, along with clinical documentation, complete medical histories and complete medical records. Personal information, including names, addresses and contact details, as well as blood test results, biometrics, and treatment records, all of which can be used to identify the individual.

A significant amount of information was exposed in an era of intensifying cyber threats, which highlights the profound consequences of poorly configured healthcare infrastructure. There has been an increasing number of breaches that illustrate the magnitude of the problem. BlackCat/ALPHV ransomware group has claimed responsibility for a devastating attack on Change Healthcare, where Optum, the parent company of UnitedHealth Group, has reportedly paid $22 million in ransom in exchange for the promise of deleting stolen data.

There was a twist in the crime ecosystem when BlackCat abruptly shut down, retaining none of the payments, but sending the data to an affiliate of the RansomHub ransomware group, which demanded a second ransom for the data in an attempt to secure payment. No second payment was received, and the breach grew in magnitude as each disclosure was made. Initially logged with the U.S. Health and Human Services (HHS) officials had initially estimated that the infection affected 500 people, but by July 2025, it had reached 100 million, then 190 million, and finally 192.7 million individuals.

These staggering figures highlight why healthcare remains a prime target for ransomware operators: if critical hospital systems fail to function correctly, downtime threatens not only revenue and reputations, but the lives of patients as well. Several other vulnerabilities compound the risk, including ransomware, since medical IoT devices are already vulnerable to compromise, which poses a threat to life-sustaining systems like heart monitors and infusion pumps. 

Telehealth platforms, on the other hand, extend the attack surface by routing sensitive consultations over the internet, thereby increasing the scope of potential attacks. In India, these global pressures are matched by local challenges, including outdated legacy systems, a lack of cybersecurity expertise, and a still-developing regulatory framework. 

Healthcare providers rely on a patchwork of frameworks in order to protect themselves from cybersecurity threats since there is no unified national healthcare cybersecurity law, including the Information Technology Act, SPDI Rules, and the Digital Personal Data Protection Act, which has not been enforced yet.

In their view, this lack of cohesion leaves organisations ill-equipped for future threats, particularly smaller companies with limited budgets and under-resourced security departments. In order to address these gaps, there is a partnership between the Data Security Council of India and the Healthcare Information and Management Systems Society (HIMSS) that aims to conduct a national cybersecurity assessment. As a result of the number of potentially exposed pieces of information that were uncovered as a result of the Serviceaide breach, it was particularly troubling. 

Depending on the individual, the data could include information such as their name, Social Security number, birth date, medical records, insurance details, prescription and treatment information, clinical notes, provider identifications, email usernames, and passwords. This information would vary by individual. As a response, Serviceaide announced that it had strengthened its security controls and was offering 12 months of complimentary credit and identity monitoring to affected individuals. 

There was an incident at Catholic Health that resulted in the disclosure that limited patient data was exposed by one of its vendors. According to the organisation's website, a formal notification letter is now being sent to potentially affected patients, and a link to the Serviceaide notice can be found on the website. No response has been received from either organisation regarding further information. 

While regulatory authorities and courts have shown little leniency in similar cases, in 2019, Puerto Rico-based Inmediata Health Group was fined $250,000 by the HHS' Office for Civil Rights (OCR) and later settled a lawsuit for more than $2.5 million with the state attorneys general and class actions plaintiffs after a misconfiguration resulted in 1.6 million patient records being exposed. As recently as last week, OCR penalised Vision Upright MRI, a small California imaging provider, for leaving medical images, including X-rays, CT scans, and MRIs, available online through an unsecured PACS server. 

A $5,000 fine and an action plan were awarded in this case, making the agency's 14th HIPAA enforcement action in 2025. The cumulative effect of these precedents illustrates that failing to secure patient information can lead to significant financial, regulatory, and reputational consequences for healthcare organisations. It has become increasingly evident that the regulatory consequences of failing to safeguard patient data are increasing as time goes on. 

Specifically, under the Health Insurance Portability and Accountability Act (HIPAA), fines can rise to millions of dollars for prolonged violations of the law, and systemic non-compliance with the law can result. For healthcare organisations, adhering to the regulations is both a financial and ethical imperative. 

Data from the U.S. As shown by the Department of Health and Human Services' Office for Civil Rights (OCR), enforcement activity has been steadily increasing over the past decade, with the year 2022 marking a record number of penalties imposed. OCR's Right of Access Initiative, launched in 2019, aims to curb providers who fail to provide patients with timely access to their medical records in a timely manner. 

It has contributed a great deal to the increase in penalties. There were 46 penalties issued for such violations between September 2019 and December 2023 as a result of enforcement activity. Enforcement activity continued high in 2024, as OCR closed 22 investigations with fines, even though only 16 of those were formally announced during that year. The momentum continues into 2025, bolstered by an increased enforcement focus on the HIPAA Security Rule's risk analysis provision, traditionally the most common cause of noncompliance. 

 Almost ten investigations have already been closed by OCR with financial penalties due to risk analysis failures as of May 31, 2025, indicating the agency's sharpened effort to reduce the backlog of data breach cases while holding covered entities accountable for their failures. It is a stark reminder that the healthcare sector stands at a crossroads between technology, patient care, and national security right now as a result of the increasing wave of cyberattacks that have been perpetrated against healthcare organisations. 

 Hospitals and medical networks are increasingly becoming increasingly dependent on the use of digital technologies, which means every exposed database, misconfigured system, or compromised vendor creates a greater opportunity for adversaries with ever greater resources, organisation, and determination to attack them. In the absence of decisive investments in cybersecurity infrastructure, workforce training, and stronger regulatory frameworks, experts warn that breaches will not only persist but will intensify in the future. 

A growing digitisation of healthcare in India makes the stakes even higher: the ability to preserve patient trust, ensure continuity of care, and safeguard sensitive health data is what will determine if digital innovation becomes a valuable asset or a liability, particularly in this country. In the big picture, it is also obvious that cybersecurity is no longer a technical afterthought but has evolved into a pillar of healthcare resilience, where failure has a cost that goes far beyond fines and penalties, and concerns involving patient safety as well as the lives of people involved.

CISA, FBI Issue Alert Over Rising Interlock Ransomware Attacks on Critical Infrastructure

 

The Cybersecurity and Infrastructure Security Agency (CISA) and the Federal Bureau of Investigation (FBI) have raised an alarm over an increase in ransomware activity linked to the Interlock gang. The advisory, released on Tuesday in collaboration with the Department of Health and Human Services (HHS) and the Multi-State Information Sharing and Analysis Center (MS-ISAC), warns that the group is actively targeting businesses and critical infrastructure in double extortion attacks.

The alert includes indicators of compromise (IOCs) gathered from recent investigations—some as recent as June 2025—and outlines protective measures for network defenders.

Emerging in September 2024, Interlock is a relatively new but rapidly growing ransomware operation. It has launched attacks across various global sectors, with a particular focus on healthcare. The gang has previously been connected to ClickFix intrusions, where they impersonated IT utilities to breach networks, and to malware campaigns using a remote access trojan (RAT) known as NodeSnake, particularly affecting U.K. universities.

The group recently claimed responsibility for cyberattacks on DaVita, a Fortune 500 kidney care company, leaking 1.5 terabytes of stolen data, and Kettering Health, a major healthcare provider with over 120 outpatient locations and more than 15,000 employees.

According to the FBI, the Interlock gang has been observed using unusual methods to infiltrate systems.

"FBI observed actors obtaining initial access via drive-by download from compromised legitimate websites, which is an uncommon method among ransomware groups," the advisory notes.

The gang uses a double extortion model—first stealing and then encrypting victims’ data—forcing organizations to pay not just to restore systems but also to prevent public data leaks.

‘FileFix’ Malware Trick Amplifies Interlock Ransomware Threat With Evolved Attack Tactic

 

Cybersecurity researchers have identified a dangerous new twist to the notorious ClickFix malware tactic. The evolved variant—called FileFix—is now being weaponized in active ransomware campaigns, further advancing the threat landscape.

ClickFix typically lures users by showing them a bogus issue—like a fake CAPTCHA or a misleading virus alert—and then offers a “solution” that involves copying and pasting a command from a compromised website into the Windows Run dialog. This command often triggers the download and execution of malicious software.

However, the new FileFix technique modifies that approach. Instead of using the Run command, it instructs users to paste a string into the File Explorer address bar. Though it appears as a legitimate file path, the string is actually a disguised PowerShell command, cleverly masked using comment syntax.

In recent attacks observed in the wild, executing this PowerShell string installs a PHP-based version of the Interlock Remote Access Trojan (RAT). Once active, the RAT performs a range of actions—scanning system and network configurations, identifying backup systems, navigating through local file directories, probing Active Directory environments, and even inspecting domain controllers.

Eventually, the RAT leads to the deployment of the Interlock ransomware encryptor.

Interlock first appeared in September 2024 and was publicly detected by November the same year. It stood out by targeting both Windows and FreeBSD systems. Some high-profile victims include Wayne County (Michigan), Texas Tech University Health Sciences Center, Heritage Bank & McCormick–Priore, and Kettering Health.

The ransomware employs the typical double extortion approach—stealing sensitive data before locking systems with encryption to demand ransom.

As of mid-2025, Interlock has been linked to at least 14 confirmed incidents, with healthcare entities making up about one-third of the total. This shift in delivery method suggests active development of the malware and underscores its ongoing threat to global organizations.

Qilin Ransomware Outfit Claims Credit for Lee Enterprises Breach

 

The Lee Enterprises attack that caused disruptions on February 3 has been linked to the Qilin ransomware group, which has released samples of data they claim were stolen from the enterprise. The ransomware actors have now threatened to release all of the allegedly stolen material unless a ransom demand is fulfilled.

The US-based media firm Lee Enterprises owns and runs 350 magazines, 77 daily newspapers, digital media platforms, and marketing services. The company's internet viewership reaches tens of millions each month, and its main concentration is local news and advertising.

In a report with the Securities and Exchange Commission (SEC) earlier this month, the company disclosed that it was subjected to a cyberattack on February 3, 2025, resulting in major operational disruption. Threat analysts discovered that the outage created serious issues, including lost access to internal systems and cloud storage, as well as non-functioning corporate VPNs.

A week later, Lee Enterprises filed a new statement with the SEC, stating that the attackers "encrypted critical applications and exfiltrated certain files," implying that they had been targeted by ransomware. 

Earlier this week, Qilin ransomware added Lee Enterprises to its dark web extortion site, publishing samples of allegedly stolen data such as government ID scans, non-disclosure agreements, financial spreadsheets, contracts/agreements, and other private papers reportedly stolen from the company. 

Evolution of Qilin ransomware

Despite not being one of the most active ransomware groups, Qilin has advanced significantly since being introduced in August 2022 under the alias "Agenda.”

In the years that followed, the cybercriminals claimed hundreds of victims, with prominent examples including automotive manufacturer Yangfeng, Australia's Court Services Victoria, and many major NHS hospitals in London. 

In terms of technical evolution, Qilin delivered a Linux (VMware ESXi) variation in December 2023, began deploying a custom Chrome credentials stealer in August 2024, and launched a Rust-based data locker with stronger encryption and better evasion in October. 

Microsoft released a report last year claiming that the infamous members of the hacking group known as "Scattered Spider" had started using the Qilin ransomware in their attacks.

Play Ransomware: A Rising Global Cybersecurity Threat

 


Play ransomware, also known as Balloonfly or PlayCrypt, has become a significant cybersecurity threat since its emergence in June 2022. Responsible for over 300 global attacks, this ransomware employs a double extortion model — stealing sensitive data before encrypting files and appending them with the ".PLAY" extension. 

Victims are pressured to pay ransoms to recover their data and prevent its public release, making Play ransomware particularly dangerous for organizations worldwide. 

Recent investigations have revealed possible connections between Play ransomware and the North Korean-linked Andariel group. Research by cybersecurity firm AhnLab suggests that Andariel utilizes malware like Sliver and DTrack for reconnaissance and data theft prior to deploying ransomware attacks. The group's history with advanced ransomware strains such as SHATTEREDGLASS and Maui highlights the increasing sophistication of Play ransomware operations. Exploitation of Security Vulnerabilities Play ransomware exploits vulnerabilities in widely used systems to gain unauthorized access. Notable targets include:
  • ProxyNotShell (CVE-2022-41040, CVE-2022-41082): Flaws in Microsoft Exchange Server exploited for initial network infiltration.
  • FortiOS Vulnerabilities (CVE-2020-12812, CVE-2018-13379): Security gaps in Fortinet products leveraged for unauthorized access.
By exploiting these vulnerabilities and using compromised credentials, attackers can bypass detection and establish control over targeted networks. 
  
Play Ransomware Attack Lifecycle 
 
Play ransomware operators follow a structured, multi-phase attack methodology:
  • Reconnaissance: Tools like NetScan and AdFind are used to map networks and gather critical system information.
  • Privilege Escalation: Attackers employ scripts such as WinPEAS to exploit vulnerabilities and obtain administrative privileges.
  • Credential Theft: Tools like Mimikatz extract sensitive login information, enabling deeper network penetration.
  • Persistence and Lateral Movement: Remote access tools like AnyDesk and proxy utilities like Plink are used to maintain control and spread malware. Additional tools, such as Cobalt Strike and PsExec, facilitate lateral movement across networks.
  • Defense Evasion: Security programs are disabled using tools like Process Hacker to avoid detection.
  • Data Exfiltration: Files are compressed with WinRAR and transferred using WinSCP before encryption begins.
  • File Encryption and Ransom Demand: Files are encrypted and appended with the ".PLAY" extension. Victims receive a ransom note titled "ReadMe.txt", providing negotiation instructions and a Tor link for secure communication.
Mitigation Strategies Against Play Ransomware 
 
Organizations can reduce the risk of Play ransomware attacks by adopting proactive cybersecurity measures, including:
  • Patch Management: Regularly updating and patching known system vulnerabilities.
  • Advanced Security Protocols: Implementing robust endpoint detection and response (EDR) solutions.
  • Access Control: Strengthening authentication methods and restricting privileged access.
  • Employee Awareness: Conducting cybersecurity training to recognize phishing and social engineering attacks.
  • Data Backup: Maintaining secure, offline backups to enable data recovery without paying ransom demands.

Its sophisticated attack methods, exploitation of known vulnerabilities, and suspected collaboration with nation-state actors make it a serious global concern. Proactive cybersecurity strategies and heightened vigilance are essential to protect organizations from this evolving threat.

The City of Columbus faces a major cybersecurity threat due to a hacking group Rhysida’s claims of stealing a massive 6.5 terabytes of sensitive information. The data heist happened after a ransomware attack on July 18 that forced the city to close down various online operations. 

Ransomware attack 

Mayor Andrew Ginther acknowledged the attack but didn’t disclose the group and the type of data compromised, only saying the attack came from an ‘established and sophisticated threat actor operating overseas.” 

Although the IT department was able to stop the hackers from encrypting the data, the hackers still got the most of it. Claiming responsibility for the attack, Rhysida is auctioning the stolen data on a dark website for sale. 

Hackers ask for Bitcoin as ransom

The ransom demand is 30 Bitcoin, which comes to around $1.9 million. The data for sale includes databases and city video camera access. The hackers promise buyers full ownership, and reselling is not allowed. In earlier attacks, if Rhysida couldn't find a buyer, they just leaked the data publicly. 

The mayor’s office is currently mute about the ongoing investigation. However, they have taken measures to save impacted employees by providing Experian credit monitoring services. The safety step extends to the whole city, judge employees, and Franklin County Municipal Court clerk. 

The mayor stressed that the threat actors’ main goal was to churn out as much money as possible, and the city is improving its cybersecurity infrastructure to avoid future attacks.

Use of Double Extortion 

According to experts, Rhysdia’s action aligns with a strategy called “double extortion.” It suggests the threat actors extracted the sensitive data before starting the encryption process. Even after the city stopped the encryption, Rhysida may still have important data. However, experts also said that Rhysida has a history of exaggerating the volume of stolen data they have claimed.

At present, the city is working to limit the crisis, the Columbus city residents await more updates and hope for an answer that prevents their sensitive data. 

“Even before the auction, some city employees were already falling victim to compromised data. Brian Steel, president of the local branch of the Fraternal Order of Police, confirmed to NBC4 that at least 12 Columbus police officers had their bank accounts hacked. However, there’s no evidence to connect this as a direct symptom of Rhysida’s attack,” reports NBC4. 

Ransomware Gangs are Evolving: Cryptocurrency Flaws Could be Their Next Target


Dallas City Government, in May 2023, faced a ransomware attack which resulted in the temporary halt in their operations which included hearings, trial and jury duty and the closure of the Dallas Municipal Court Building. 

The attack further impacted police activities, as overstretched resources made it more difficult to implement initiatives like summer youth programs. Threats to publish private information, court cases, prisoner identities, and official papers were made by the criminals.

One may think that cyberattack on city government would be a headline news, however, this year has seen a number of such instances that any mere attack is just another common topic of discussion. A notable exception was the vulnerability exploitation of a Moveit file transfer app in May and June 2023 that led to data theft from hundreds of organizations across the world, including British Airways, the BBC and the chemist chain Boots. 

Apparently, over the past years the ransom payments have doubled to US$1.5 million, with the big-profit organizations paying the highest price. A British cybersecurity company called Sophos discovered that the average ransomware payment increased from US$812,000 the year before. At US$2.1 million, the average payment made by UK organizations in 2023 was considerably greater than the global average.

While ten years ago this was no more than a theoretical possibility and niche threat, but ransomware has now gained a wide acknowledgment as a major threat and challenge to modern society. Its rapid evolution, which has fueled crime and done enormous harm has raised serious concerns. 

The "business model" for ransomware has evolved as, for example, malware attack vectors, negotiation tactics, and criminal enterprise structure have all advanced.

Criminals are now expected to adapt to their strategies and cause digital catastrophe for years to come. In order to combat the long-term threat, it is crucial to examine the ransomware threat and anticipate these strategies.

What is Ransomware?

Until now, crypto has relied on a so-called “proof-of-work” consensus mechanism where the authorization of transactions include solving a complicated math problem (the work) to authorize transactions. This strategy is not long-term viable since it leads to unnecessary large-scale energy use and duplication of effort.

A "proof-of-stake" consensus method is the alternative, which is increasingly becoming a reality. In this case, validators who have staked money and receive compensation for validating transactions approve transactions. A financial stake takes the place of the role played by ineffective work. While this solves the energy issue, it also means that substantial sums of staked money are required to validate crypto-transactions.

Rapid Ransomware Dwell Time and Persistent RDP Vulnerabilities

The dwell period of ransomware hackers has decreased to just 5 days, a noteworthy trend in the constantly changing world of cyber dangers that demands prompt response. The urgent necessity for stronger cybersecurity measures is highlighted by the quick infiltration and encryption timeframe as well as the ongoing use of Remote Desktop Protocol (RDP).

The dwell time, which measures how long an unauthorized actor stays within a hacked system before launching a cyberattack, has substantially lowered to just 5 days, according to a report by BleepingComputer. This is a considerable decrease from the prior average of 18 days, indicating that threat actors are getting better at quickly entering target networks and deploying their destructive payloads.

The report also highlights the persistent use of Remote Desktop Protocol (RDP) as a primary entry point for ransomware attacks. Despite numerous warnings and documented vulnerabilities, RDP remains widely used due to its convenience in enabling remote access. Security experts have long cautioned against RDP's risks, emphasizing its susceptibility to brute force attacks and the potential for unauthorized entry.

A study by Sophos echoes these concerns, revealing that RDP-related attacks remain a prevalent threat vector. Cybercriminals exploit misconfigured RDP services and weak passwords to gain unauthorized access to systems, making them ripe targets for ransomware deployment. The consequences of such attacks can be devastating, leading to data breaches, operational disruptions, and substantial financial losses.

The widespread reliance on RDP is concerning, given the increasing sophistication of ransomware attacks. Attackers are employing various tactics, such as double extortion, where they not only encrypt sensitive data but also threaten to leak it unless a ransom is paid. This creates a multifaceted dilemma for organizations, forcing them to not only recover their systems but also mitigate potential reputational damage.

The security community has also discovered new RDP-related vulnerabilities, according to The Hacker News. These flaws include things like unreliable encryption, a lack of two-factor authentication, and vulnerability to 'pass-the-hash' attacks. The critical need for businesses to review their remote access policies and make investments in safer substitutes is further highlighted by these fundamental shortcomings.

Organizations must take a multifaceted approach to improve their cybersecurity defenses in order to counter these expanding threats. This entails putting in place tight access controls, enforcing strict password guidelines, and routinely patching and updating systems. Ransomware attacks can be considerably reduced with the use of more secure remote access technologies in place of RDP and thorough employee training.
